Description: Molding
variations in this part as used in the OMS Buoyancy Control System
(BCS) could cause cracking or breakage of part, resulting in rapid
loss of buoyancy, creating a potential drowning hazard.
Incidents/Injuries: There have been reported of 19 failures. These
failures were discovered during inspection, with no reports of
injuries or death. There are a total of 19,790 BCS Seal Rings
(Item # BCA 500) installed in 5730 BCS mfg'd from May 11, 2006
through August 1 2007.
While this variation is not present in every Seal Ring (Item #
BCA 500), it would be difficult to determine in the field which
Seal Rings are suspect. Therefore we are requiring that all Seal
Rings be updated with new assemblies. All NEW seal rings are
equipped with reinforcement at critical areas. |

Dive Rite Wings
Description:The
over pressure valve springs in the diving equipment could rust and
fail allowing the buoyancy compensator devices to leak, posing a
drowning hazard to divers.
The Dive Rite Wings affected in
this recall include the following models Travel, Venture, Rec,
Trek, Classic, Nomad and Super Wings. The wings were sold in red,
blue or black. Springs were used on wings that have an opaque
white or blue tinted bladder and welded in flanges. Wings with a
black bladder are not affected. Only the Dive Rite wings that have
a serial number range from 42000 through 72000 are affected by
this recall. The serial numbers can be found on a white tag
attached along a seam of the wing.
Remedy: Consumers
should immediately stop using recalled diving equipment and return
it to an authorized distributor or dealer for a free replacement
of the overpressure valve spring.

OMS Buoyancy Compensators
Description:The
buoyancy compensator seal ring could crack. The compensators were
sold in black or red. “OMS” is printed on the front inside of the
compensators. Item and serial numbers are printed on the warning
label located in the non-inflation area of the buoyancy
compensator. A list
of serial numbers included
in this recall is available from the firm. This recall involves
buoyancy compensators with the following model numbers.

OMS
announces recall of Surface Marker Buoys (SMB'S) and Lift
Bags.
Ocean Management
Systems, Inc. (OMS) of Middletown, NY, is re- calling
approximately (287) Surface Marker Buoys and (77) Lift Bags sold
from March 03, 2003 to June 20, 2003 due to malfunctioning
Overpressure (OPV) Valves. Molding variations in the male
threaded component of the overpressure valve (OPV) used in
both OMS Surface Marker buoys (SMB) and OMS Lift Bags, could
cause the pull dump part (female threaded part) of the OPV to
separate from the male thread ed portion of the (OPV). This could
cause deflation of the Surface Marker Buoy and lift bags.
Both the SMB's and Lift
bags are not personal flotation devices and OMS has not had
any reports of injuries due to this malfunction.

Product Safety Recall : Part
number V 112 
Attn: Dive stores and
end users. This cap as shown is under recall. Please take this cap
out of service immediately as it can non-passively fail if over
torqued.
These caps will be
replaced immediately at no charge by OMS upon receipt of the
previous model V112 with a new "rounded cap" 
The new "rounded" cap
is to be hand tightened, and has a 6mm Hex head for measured
torqueing purposes (110 inch lbs).

CPSC, Sherwood SCUBA LLC Announce Recall of Maximus Regulators
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product
Safety Commission (CPSC), Sherwood SCUBA LLC, of Irvine,
Calif., is recalling about 14,000 Maximus SCUBA regulators.
The second stage orifices on these regulators can be cracked,
bent or broken. This can result in a loss of breathing air
underwater.
Sherwood SCUBA has received seven reports of
broken second stage orifices on these regulators. Two of the
regulators broke during dives underwater. No injuries were
reported.
The Maximus regulators included in this recall
have the following model, serial numbers and connection
types:
